Diabetes - Community Diabetes Regional Outreach Partnership
A coalition of six counties in upstate New York committed to raising awareness and educating the community about diabetes risk and prevention. Emergency Meals Program:
Provides funds necessary to purchase emergency meals for our county Meals-On-Wheels clients. Clients use these when normal delivery is cancelled due to weather conditions such as a snowstorm for example.

Health Professional Shortage Areas (HPSA's):
In order to provide our local physician community with the opportunity for enhanced reimbursement rates the Network will complete applications for both a primary care and mental health HPSA.
Healthy Start Partnership
The Healthy Start Partnership of Herkimer County is a coalition of health and nutrition professionals who are working to promote healthy weights in childbearing women and their infants. Agencies involved include Community Maternity Services, Cornell Cooperative Extension of Herkimer County, Herkimer County HealthNet, Herkimer County Public Health, Herkimer –Madison WIC, Mid-York Child Care Coordinating Council, and the Mohawk Valley Perinatal Network.

Move 4 Life
Move 4 Life is a program that recognizes that people come in all shapes, sizes and physical abilities, but no matter where a person is, can be tailored to meet their individual goals. This fun and interactive 8-week program is designed to help increase or maintain an individual’s physical activity level in order to enjoy the benefits of regular physical activity including: lower stress levels, weight management, better sleeping patterns and increased energy.
Parents-As-Teachers Program:
Parents as Teachers (PAT) is a parenting program based on the philosophy of providing parents with child development knowledge and parenting support. PAT has the following goals:
- Increase parent knowledge of early childhood development and improve parenting practices
- Provide early detection of developmental delays and health issues
- Prevent child abuse and neglect
- Increase children's school readiness and school success

Speakers Bureau
The Speakers Bureau provides an opportunity to disseminate health information to the community on a wide variety of topics. Herkimer County HealthNet has enlisted local experts to speak on a wide variety of topics regarding health and human services. The Speakers Bureau will enhance the knowledge of Herkimer County residents on a variety of health topics with the ultimate goal of improving the health and well being of Herkimer County Residents. Teen Internet Crisis Hotline:
The Network worked collaboratively with others on finding an innovative method of having teens help teens with the many issues they face.
